Abstract
commonest endocrine dx. It often coexists with obesity, dyslipidaemia and hypertension. A key part of management is weight loss therapy. This includes lifestyle, dietary counsel and sometimes medications. This study was to measure how much weight patients were able to lose with different methods. Obese (BMI >30kg/m2) type 2 DM patients of two clinics in Lagos (public and private) covered by the authors were retrieved and biodata and weight records perused. Other information such as the means to weight loss deployed. 52 patient records (98 were female) were studied. Mean height 1.65m, mean weight 108kg (mean BMI of 39.7kg/m2). The weight loss method: dietary counsel given to all patients, 1-3 weight-reducing drugs including orlistat, topiramate, SGLT-2 inhibitors and GLP-1RAs. Mean weight loss was 4.96kg (range -30 to +28kg) for those managed with weight loss drugs. 108 (71.4%) of patients lost weight over a period 1-3 years. Weight loss of range 1-30kg was seen with a mean weight loss of 12.4kg in this group. The 28.6% who did not lose weight gained 1-28kg, mean weight gain 4.16kg over the same period. Those who experienced the greatest weight loss
were on multiple weight loss drug therapy in addition to diet. Only 3 patients had bariatric surgery for weight loss which was beneficial in one (lost 31kg), equivocal in one (only transient weight loss and regained most of weight) and not measurable in the 3rd (lost to follow up). Obesity is a very difficult condition to treat in patients with DM and most often
only modest effects are seen with lifestyle and diet, while more aggressive care with weight loss treatment (combination drug therapy and surgery) is often required to be deployed for better results. Obese DM patients require more active and aggressive intervention for improved metabolic control and obesity should be actively managed with diet, exercise, medications, and surgery if necessary/indicated.
