Concomitant chronic kidney diseases (CKD) and diabetes mellitus foot syndrome (DMFS) in a tertiary health care centre in Nigeria

dc.description.abstractBackground Chronic complications of diabetes mellitus often co-exist due to a common pathophysiologic mechanism. The presence of CKD in a patient with DMFS can affect management and adversely influence patient’s clinical outcome. Objective To measure the prevalence of CKD amongst in- patients admitted for DMFS. Methods Data from 33 from of 50 patients with DMFS and complete investigations in a prospective study was analysed. They include the age, sex, duration and type of diabetes, presence and duration of hypertension, smoking history and serum creatinine levels. The medical nutrition in renal disease equation (MDRD) was used to calculate the GFR. CKD was defined as calculated GFR < 60 mls/min. The results are expressed as mean SD. Results There were 24(73%) males and 9 (27%) females. The mean age was 57.2  12.4 years. All but 2 patients had type 2 DM. The mean duration of diabetes was 8.9  7.6 years. Of the 33 patients 17(52%) were hypertensive, 10 (30%) were past smokers. None was currently a smoker. 13(39%) patients had CKD (i.e. GFR < 60 mls/min) when the MDRD equation was used to calculate the GFR. 6(46%) of these 13 patients with CKD were also hypertensive. Conclusion. There is a high prevalence of CKD in patients with DMFS. Thus assessment of GFR should be routinely done in patients with DMFS.en_US
