Provision and Utilization of Health Care Services as Correlates of Incidence of Occupational Diseases Among Textile Workers in Lagos State, Nigeria

Abstract
The study investigated the provision of healthcare services by the employers, its utilization by the textile workers and incidence of occupational diseases among the workers in Lagos State. Diseases are bronchitis, conjunctivitis, boils, fatigue, and earache. Ex-post facto research design was used for the study. 1,200 textile workers from 18(51.4%) out of 35 registered textile industries comprised, the sample for the study. Sample was selected through random sampling technique. Data collection instruments were to questionnaires; Health Services Provision (HSPQ) (R = 0.92) and health service utilization (HSUQ) (R = 0.94). A checklist with inter rater's reliability (r = 0.70) was used to checkmark the provision and utilization health care service at work place. The variables investigated were provision and utilization of healthcare services (independent variables) and incidence of bles) and incidence of occupational disease (dependent variable). Medical records of the 18 company-based clinics were reviewed from1998-2000 that provided data on the incidence of occupational diseases from the diagnosis. The descriptive statistics of frequency counts, percentages and step-wise multiple regression were used to provide answers to the research questions at 0.05 as level of significance. Responses from the HSPQ & HSUQ revealed average provision of healthcare services of 38.7% and utilization of 28.6%. The two independent variables correlated 0.9773 with the dependent variable (incidence of occupational diseases). They were found to have contributed 95.51% (R = 0.9551) to the variance of the dependent variable. Also, provision and utilization of healthcare services could predict the incidence of occupational diseases among textile workers (B = 4.18; t = 3.24; p<0.05) and (B = 3.04, t = 2.91; P<0.05) respectively. The average incidence of occupational diseases anong textile workers was 11.3% (113/1000). Based on these firidings, it was concluded that the employers substandard and incomplete provision of health care services for the workers and this (contributed to high incidence of occupational diseases (113/1000) among textile workers in Lagos State. It was therefore recommended that conscious effort should be made by the employers to ensure that education is intensified through established safety and disaster team to supply the workers with the relevant health promotion information in the languages they can understand.
