Abstract
Background: Type 2 diabetes mellitus has been on the rise amongst children and adolescents over the last two decades. Obesity and a family history of diabetes are the most common factors associated with this trend.
Objective: To present a case report of type 2 diabetes in an overweight teenager with a strong family history of diabetes mellitus.
Method: We describe a case of type 2 diabetes who presented with progressive weight loss, polyuria and polydipsia. We also discuss the results of investigations done, treatment modalities involved and management outcomes.
Results: A 14-year-old boy who was dehydrated, had diabetic ketoacidosis, with evidence of chronic hyperglycaemia, and residual pancreatic beta-cell function on investigation. Treatment included prompt rehydration, dietary therapy and a high total daily dose of insulin to achieve glycaemic control over two weeks on admission. He was discharged and subsequently had a progressive reduction in total daily insulin dose due to tight glycaemic control. Treatment was changed to metformin and continued lifestyle modification with which he maintained good glycaemic control.
Conclusion: Overweight and obese Nigerian adolescents with a strong family history of diabetes can develop type 2 diabetes mellitus. Lifestyle modification is essential to achieve and maintain good glycaemic control in adolescents with type 2 diabetes.
